• Sensory Motor History (5 pages) - Complete & Return
• Clinic Maps (2 pages) - Keep
• Parent Book List (1 page) - Keep
• Notice of Privacy Practices (3 pages) - Keep

Does NTTI take Health Insurance?

Yes, NTTI files treatment claims on behalf of the patient as an Out-of-Network providers with United Healthcare, Blue Cross/Blue Shield, Aetna, Cigna and most PHCS plans.

This means the patient only pays their responsibiltiy at the time of service while NTTI collects the insurance responsibity directy from the insurance carrier.
